Make A Referral
PRIVATE PATIENTS
If you are a clinician wanting to make a referral for a private patient then you can request a scan by completing a secure online referral using your computer, tablet or smartphone.
We encourage you to use this as it provides a safe and secure method of ensuring we have the correct information to provide an appointment for your patient quickly. We will ring your patient within one working day to arrange an appointment with them.
The first time you use this service, you should register your contact details with us, and we will issue you with a username which you can use each time you request a scan. This will make it easier to request scans in the future and won’t need to enter your contact details each time.
Please provide us with an email address and mobile phone number for communication purposes. We will send you alerts using SMS to your mobile phone to inform you of when your patient has booked their appointment with us so that you can schedule a follow-up consultation.
You will also receive an SMS when your patient has had their scan and the report is ready to be downloaded from our portal. If you wish, you can specify your secretary’s mobile phone or email address for notifications.
We also accept patient referrals by fax or email. Please download a request form below if you wish to use fax or email to send a request
